“HOME CARE PACKS”
Janet Welton, a senior nurse with the Annapolis Valley Palliative Care team, had a vision. She knew that patients being cared for at Hospice were provided with practical care items from soaker pads to lip balm, but not patients who chose to die at home. She wanted to change this, to support patients wherever they chose to die, providing them equally with care and comfort.
And, so, the idea of Home Care Packs was born. These packs would be filled with essential personal care items, carefully selected to meet end-of-life needs. Janet, supported by other team members and the Valley Hospice Foundation, worked towards making this vision a reality. The Packs would be filled with everything from toothettes, lip balm, mouthwash, slider sheets, soaker pads, incontinence pads and briefs, skin cream, wipes, and a comfort card with a crocheted heart.
As of March 2025, thanks to the generous donations of our caring community, palliative patients being cared for at home now receive a Home Care Pack – and a caring vision has been realized.
The mandate of Valley Hospice Foundation is to support Hospice and Palliative Care across all care settings. We work with the care teams to understand the needs of their patients and find ways to support those needs. The Foundation extends support through the Annapolis Valley Palliative Care Program that serves communities in Kings and Annapolis Counties.
Each Home Care Pack costs $100, with an estimated annual cost of $10,000.
These Home Care Packs are funded entirely from generous donations.
